For The First Time In Pakistan
Liquid Based Cytology For Cervical Smears

Liquid Based cytology (LBC) was first introduced in 1999, and has now become the standard methodology for cytology preparations throughout the developed world. This technique has been adopted by 100% of the laboratories in UK, approx. 90% in USA, 70% in Europe and has also been adopted by major labs in India as well as in the Middle East. Liaquat National Hospital Laboratory Service is the first one in Pakistan (and currently the only one) to offer this technique, for reporting of cervical smears.

What is LBC? It is the preparation of thin layer of cells on a microscopic slide, suitable for the diagnosis of cytological abnormalities. According to a recent large survey conducted by the College of American Pathologists, LBC has several advantages over conventional Pap smear.

  • Increased sensitivity
  • Significant reduction of unsatisfactory samples
  • Decrease number of ambiguous reports
  • No poorly preserved samples.
  • Ancillary studies, like detection of HPV can be performed on the same sample.
  • Methodology:

    Thin-Prep by Hologic (FDA approved)

    The sample is collected by the usual method using a spatula/ broom. The broom is then rinsed into the vial containing fixative. This vial is sent to the lab for processing. (Sampling kit is provided by the Lab.) The smears prepared have a clean background, even distribution and monolayer sheets of cells, which can be accurately assessed and interpreted.

    GESTATIONAL DIABETES CLINIC FOR PREGNANCY

    GDM is a type of Diabetes that occurs during pregnancy affecting approximately 5% of all pregnancies. The detection of GDM is important because of its associated maternal and fetal complications.

    Women who are at high risk for gestational diabetes generally are screened early in their pregnancy, sometimes during their first prenatal visit. Risk factors include family history of type 2 (adult onset) diabetes, prior birth of a baby 9 pounds or larger, gestational diabetes in an earlier pregnancy, obesity and being age 35 or older.

    This clinic is for women who are diagnosed with gestational diabetes or women who are diabetic and become pregnant.
    A diabetes educator, a registered dietitian and an Endocrinologist staff the clinic.

    During her care at the clinic, the patient will receive the following services:

    - Gestational diabetes education plan with goals that are developed with the patient/family.
    - Nursing Consultation
    - glucose (sugar) monitoring
    - Breast feeding, exercise, diet and general information about gestational diabetes and insulin instruction if needed.
    - Medical Nutrition Therapy, including diet history, meal preparation, assessment and planning.
    - Endocrinologist Consultation, including counseling and coordination of care with the primary care doctor.

    OBESITY

    Obesity is defined as an abnormal or excessive fat accumulation that presents a risk to health. Obesity is a major risk factor for a number of chronic diseases, including diabetes, hypertension, cardiovascular diseases, backache, knee joints problem and certain hormonal problems in women.

    Apart from medical issues, obesity is responsible for psychological and social consequences especially in females. Once considered a problem only in high income countries, overweight and obesity are now dramatically on the rise in low- and middle-income countries, particularly in both rural and urban settings.

    Considering obesity a major public health problem, Department of Diabetes, Endocrinology and Metabolism has designed to provide nutritional care counseling, non-pharmacologic and pharmacologic interventions to patients for management of obesity. The attempt is to guide the patient towards an optimum nutritional status and a healthier lifestyle that will benefit not only the patients but also their families.

    GROWTH CLINIC

    Parents and children, especially teenagers, often worry when they notice that they are not as tall as many of their peers and classmates of the same age.  There are many medical causes for being short and having poor growth, including growth hormone deficiency, hypothyroidism, Turner syndrome, inflammatory bowel disease, kidney problems, malnutrition, etc. They may have genetic short stature or a constitutional growth delay.

    Department of Endocrinology and Diabetes, LNH is starting a specialized clinic for children and adolescent with disorders of growth. The aim behind this service is to screen for the treatable causes at the right which is the only solution to the problem.

    The observation/factors that can make one realize there is some problems with the growth include:

    - If the child appears shorter than the other children of same age group in the surrounding or siblings
    - The height of the child remain static for long period
    - The body is not maturing as it is expected for that age
    - The child is having precocious puberty signs that is breast development, body hairs or menses in girls and beard/moustache growth or increased penile/testes size in boys which is happening before right age
    - Underweight or excessive weight gain

    SUB FERTILITY CLINIC (Infertility Clinic)

    One in 6 couples experience some difficulty in starting a family. They need proper counseling & treatment by experts.     The department of Obstetric & Gynecology, Liaquat National Hospital Karachi runs a regular fertility clinic in both morning and afternoon sessions throughout the week.

    - Detailed history of females and males.
    - Detail examination of females.
    - Hormone analysis of female is done on specific days.
    - Baseline U.S.S of pelvis where indicated.
    - Hysterosalpingogram (HSG), (which is an X-ray test that looks at the inside of the uterus and fallopian tubes and the area around them. It often is done for women who do not conceive even after six cycles of treatment.
    - Laparoscopy. (Gynaecological laparoscopy is a procedure used to look inside your abdomen (tummy) and examine your fallopian tubes, ovaries and womb).
    - Hysteroscopy.
    - Semen analysis of husband.
    - Induction of ovulation after necessary investigations.

    UROGYNAECOLOGY SERVICES

    The field of Urogynecology (a subspecialty with in Obstetrics & Gynecology) is dedicated to the treatment of women with pelvic floor dysfunctions such as urinary & fecal incontinence and pelvic organ prolapse (bulging of vagina due to bladder, uterus or rectum protrude through vagina).

    This is multidisciplinary a field which involves special services for related problem. 

    What Is Urinary Incontinence?

    Urinary incontinence is a symptom, not a disease, this means that the there are many possible causes of urinary incontinence.  The key to the treatment is identifying the specific type(s) of incontinence that a woman through careful medical interview and focused physical examination. It may also be necessary to perform a special test called Uro-dynamics to diagnose the problem. Before need for this special test simple tests are done to exclude other reasons for incontinence.

    MENOPAUSE CLINIC

    The Menopause is the complete cessation of period for atleast 1 year. This is a normal condition that occur around age of 47 years. Every woman experienced this condition at certain age. This is associated with mood changes, hot flustes & urinary problems.

    Menopause is a natural biological process, not a medical illness even so the physical & emotional symptoms of Menopause can disrupt sleep, energy & at least indirectly trigger feelings of sadness & cause the physical symptoms of Menopause, but mistaken beliefs about the menopausal transition are partly to blame for the emotional ones. Menopause does not mean the end is near, you have still got as much as half your life to go. Hormonal deficiency affect menopause will not feminity & sexuality. In fact, you may be one of the many women who find it liberating to stop worrying about period.

    The Menopausal clinic of out patient department of gynae & obs run daily by expert consultant to provide complete guidance about the condition, relevant investigation & treatment to improve quality of life.

    In menopausal clinic the team of expert sees patient, take full medical history, screening blood tests will be done to check patient’s general health as well as to check changes in hormonal level.     GYNECOLOGICAL CANCER SCREENING

    The out patient department of LNH offers gynaecological cancer screening facilities daily from Monday to Saturday. 

    The screening program include pap test, it is a simple test that can help prevent cancer of the cervix by diagnosing precancerous state. The pap test’s still the only way to detect any changes in the cells of your cervix which might develop into cancer if found early & treated, these changes will not develop into cancer. Although some what uncomfortable & embarrassing for some, the test only takes a few minutes & it could save your life. 

    A regular pap test should be done every 3 years in all sexually active patients to find changes early & treat to avoid cancer. It can find cancer of the cervix at an early stage, when women have no signs & symptoms of disease. If Smear comes positive, Colposcopy can be done & appropriate treatment can be advised. 

    If you had a hysterectomy (removal of uterus) you may still need a regular vault smear. A woman whose cervix is not completely removed or a women who had treatment for pre-cancer condition of cervix, should continue to have yearly vault smear after Hysterectomy. The vault smear facilities for the females who had hysterectomy are regularly provided. The pipelle test in case of bleeding irregularity & post menopausal bleeding to exclude & diagnose endometrial carcinoma. 

    The other screening method include breasts examination & ultrasound of pelvis. All these screening tests can be done in OPD on daily basis.
