Key Responsibilities:
- Manage critical calls related to drug information and toxicology, delivering evidence-based recommendations utilizing specialized resources.
- Oversee the Hospital Formulary lifecycle, including addition/deletion forms and identifying alternates for non-available items.
- Lead ADR logging with VigiFlow/Vigilyze and coordinate internal/external PV meetings.
- Conduct research and write for the CFB Quarterly, LNH Newsline, and Pharmacy Newsletter.
- Create SOPs and KPIs, provide training sessions and workshops on dose calculation for nursing and pharmacy staff.
- Manage P&T Committee minutes and drug recall protocols in an audit-ready environment.
Qualifications and Experience:
- Doctor of Pharmacy (Pharm-D) degree, duly registered with the Pharmacy Council, along with a minimum of 2 years of experience in a hospital pharmacy setting.
- Excellent interpersonal, communication, and writing skills, with strong problem-solving abilities.
- Proven expertise in reviewing product literature and conducting trend analysis.
- Ability to manage multiple priorities effectively and work independently with sound judgment.
